Esketamine depression treatment

A new option in the treatment of treatment-resistant depression

What is Spravato?

Spravato® (esketamine nasal spray) is an innovative medication approved in the USA and the European Union, used for the treatment of treatment-resistant depression in adults.

This means that if traditional antidepressants have not led to sufficient improvement, Spravato® may offer a new therapeutic pathway.

How does it work?

Spravato® acts via NMDA receptors, affecting the brain’s glutamatergic system.

Unlike traditional antidepressants, it is characterized by a rapid onset of action: improvement may be observed within hours to days.

Treatment is not administered alone, but with medical and nursing supervision, in a clinical setting, and with psychological support.

How is treatment carried out at our clinic?

Eligibility for therapy:

Preliminary evaluations

If your psychiatric assessment was not previously performed at Mind Klinika or Insight Medical (i.e., you are not already under the psychiatric care of our clinics), beginning therapy requires attendance at a psychiatric screening exam as follows:

A 45-minute psychiatric consultation, in person or via telemedicine; please bring any previous documentation if possible (50,000 HUF).

Psychiatric assessment and evaluation of therapy indication, issuing a pharmacy notice and prescribing medication during treatment, which may vary for each individual.

Cardiology and general medical screening is mandatory for all patients prior to initiating therapy.

Course of treatment

Treatments are conducted twice weekly, each session lasting 90 minutes.

Administration takes place on reclining beds, within screened pavilions.

Safety and supervision

Treatment is closely monitored by trained medical and nursing staff.

Throughout the process, vital parameters are monitored (e.g., blood pressure, pulse, oxygen saturation).

Each session is preceded and followed by an official, standardized checklist to ensure your safety and wellbeing.

What can patients expect?

During treatment, patients lie in a relaxed, calm environment.

Transient, mild side effects are uncommon but may occur (e.g., dizziness, dissociative experiences, increased blood pressure), which is why continuous monitoring is necessary.

After 90 minutes of observation, you may safely leave, but it’s important that you do not drive and have an escort for the rest of the day.

Who is it recommended for?

Adult patients who have not experienced sufficient improvement from at least two different antidepressant medications taken at appropriate doses and duration.

Who are willing to follow the full treatment protocol, including preliminary assessments and regular check-ups.

What does treatment-resistant depression mean?

The definition varies slightly, but most international guidelines (e.g., APA, CANMAT, NICE, EMA) define it similarly:

Treatment-resistant depression is present when:

the patient has tried two different antidepressants (from different pharmacological groups),

at adequate dosages and for an adequate period (usually at least 6–8 weeks per drug),

with proper therapeutic adherence,

and neither led to satisfactory improvement (neither complete remission nor clinically significant partial response).

Important notes

Spravato® therapy does not replace other psychotherapeutic or pharmaceutical treatments, but is intended as a supplement to them.

Successful treatment is based on close cooperation between patient and care team.

If you do not meet the indication criteria (treatment-resistant depression, i.e., at least two different antidepressant medications have been ineffective despite appropriate duration and dosage), we cannot offer participation in the therapy at our clinic.
